Our office does accept most Dental Insurance Plans, however depending on Network Levels or Plan Types, your insurance may cover at a lower percentage, or possibly deny services provided by our office. Our office staff is happy to check your Insurance Plan prior to your appointment. Please be sure to provide your insurance information at least 24 hours before your appointment time.
The best way to establish if our office is an IN or OUT of Network office is to call your insurance directly. Private Insurance companies offer numerous plans with numerous network participation requirements and rules. To ensure you receive the BEST coverage with your specific insurance plan, call your insurance directly.
Our office is able to bill some insurance plans through Oregon’s Medicaid/Medicare System (OHP).

To find out which plan your child has, check the front of their Insurance Card. If your child’s dental plan is not listed on the front of the card, or you wish to change Insurance Plans to one that is accepted by our office, please call the Customer Service phone number on the back of your card.
In order to make payment for services as convenient as possible, while at the same time maintaining operations of our office in the highest standard of comprehensive care, we offer three different payment options (see below). We will do our best to give you an accurate estimate of your total fees at the onset of your child’s treatment, however in some cases the required treatment will be more or less expensive than was originally quoted. All estimates are based on insurance information provided by the parent/guardian, and estimated coverage is not a guarantee of payment from your insurance provider.
Payment of your estimated patient share is expected at the time of service. We accept Cash, Check, Visa, Master Card, Discover, American Express, and Care Credit. A 10% discount will be credited for any accounts who do not have dental insurance.
Our office understands that the cost of dental treatment can sometimes be unexpected. In order to help ease any financial burden, we offer in-house payment plans. Each plan is customized based on the cost of your child’s treatment and the amount of months you would like to pay. There is no interest and no early termination fee if you wish to pay your account off early. You will be required to keep a debit or credit card on file for automatic monthly payments. If the account becomes delinquent due to three (3) missed payments, our office reserves the right to turn the account over to an outside collections agency. For additional information regarding this payment option, please ask a front office staff member.
We will gladly file insurance claims and accept assignment of benefits in place of payment at the time of service. You will still be responsible for any non-covered services, co-insurances, copayments at the time services are rendered. Insurance payments are determined by your insurance company at the time they receive the dental claim based on their “usual and customary” fee schedule. Your insurance company’s fee schedule may not align with our office’s charges. You may be responsible for the difference in these amounts. You are financially responsible for any charges not covered by your insurance.

To best serve our patients, we kindly ask for your appointments to be kept, or to be notified 24 hours in advance of the cancelation of an appointment. We do understand emergencies happen, and calling before missing an appointment is not always possible. If two (2) appointments are missed within a six (6) month period, you will be responsible for a $30.00 missed appointment fee (per missed appointment).
To honor those who serve, a 15% discount will be applied to any out-of-pocket cost that is incurred in our office. A parent/guardian must be employed by the US Military, Fire Department, or Police/Sherriff Station to qualify.
Dr. Jay Vaikuntam, known to his patients as “Dr. Jay”, grew up in the South of India before fulfilling his life-long dream of being a Pediatric Dentist. After obtaining his Dental license in Madras, India, he moved to the U.S. to further his Dental Education and obtain his Certificate of Pediatric Dentistry from the Rochester Eastman Institute of Oral Health in New York. Dr. Jay then spent time as an Instructor to Dental Students in the area of Pediatric Dentistry at the University of Texas Health & Science Center, and the University of Minnesota.
Dr. Jay has always had a true passion for working with children- something that is obvious to every patient and parent through his kind and caring demeanor, his patience, and his ability to make the dental office an enjoyable and fun environment. Creating and cultivating a “Dental Home” is the philosophy of Dr. Jay’s practice. The idea behind the “Dental Home” is to create a space that protects and promotes the overall health of our patients and families starting with good oral health habits at home.
In his free time, Dr. Jay enjoys spending time with his wife and two daughters, along with their two Golden Retrievers, Bella and Leiah. He also loves cooking, exploring the beautiful outdoors, and practicing photography.
Dr. Choi received his undergraduate degree in nursing from Duke University and a master’s from North Carolina State University. He received his Doctor of Dental Medicine (DMD) from Oregon Health and Science University, School of Dentistry in Portland. He continued post graduate training in a hospital dentistry residency from ECMC/University at Buffalo, followed by a pediatric dentistry residency at University of Rochester/Eastman Institute for Oral Health in New York. He also holds a black belt in Tae Kwon Do. He has volunteered in Habitat for Humanity, Give Kids A Smile, Oregon Mission of Mercy, Special Olympics Special Smiles, etc.
He is a board-certified pediatric dentist and a diplomat of the American Academy of Pediatric Dentistry. Dr. Choi brings all of his extensive training to Pediatric Dental Associates to provide the very best care for kids. As an avid learner he attends multiple continuing education courses each year to stay up to date with the latest techniques and offer his patients the most advanced care possible.
He is happily married to his wife Lorena and expecting a baby soon! He loves learning new recipes on YouTube, also spending time outdoors walking, running, and hiking.
